Required fields and form submission validation in Forms

Add an option in the Forms tab that allows designating certain fields as required before a patient can submit a form. Currently, patients can submit forms with important information missing, yet the system still marks those forms as completed — for example, credit card authorization forms where patients enter their card information but leave the expiration date blank, so the form appears completed even though essential information is missing. This creates unnecessary back-and-forth communication, additional administrative work, and potential liability concerns, particularly for documents such as informed consent forms, where a form may appear signed and completed despite missing required information. Suggested behaviour: a "Required Field" option when creating or editing forms; if a required field is left blank, the patient receives a notification identifying the missing information and is unable to submit the form until it is completed. Ideally, the system would also distinguish between forms that are fully completed and those that have been submitted with missing information. This would help ensure that important documentation is complete, reduce unnecessary follow-up communication, and improve both the patient and provider experience.

Scoring and score ranges for common assessments like PCL-5

Add scoring and score ranges for common assessments (e.g., PCL-5) under insights, including ranges for subscales or overall scale. Most competitors in the EHR space offer scoring of common assessments even at the lowest tier offerings, so the lack of even basic scoring is a major gap in utility and a concern.

Automatic scoring of administered assessments in patient notes

Automatic Scoring of Measures Add the ability to use dot phrases (like in Epic) to auto-populate scores from administered assessments into a patient note. Also add the ability to include administered assessments as an encounter and have these automatically scored as part of the progress note and/or patient chart. Add this to UpHeal functionality.

Auto-trigger form re-signatures on a schedule

Add the ability to toggle on automatic re-signing of a form at a set frequency, so signatures can be renewed on a recurring schedule (for example, GFE).

My Outcomes

hello - love all the features. I was wondering if you might be able to somehow incorporate My Outcomes (paid subsciption) in Upheal as we use it here with our students to assess our sessions. Just would be useful in one place. https://myoutcomes.com/

Insights for Assessments

Currently only GAD-7 and PHQ-9 scores are shown in insights. Can Upheal start tracking insights for other assessments like the RSE and PCL5 as well?

Counselor sign form after submitted

Is it possible to send a client a form, have them sign it, submit it, and then I can sign it as the provider by going to their account, into the Forms tab, and open their signed document? I’m not necessarily talking about doing it together in the session.

Portuguese language

Hello, would you consider having the forms in European Portuguese (PT)? Thank you!

Additional Forms

Hello, Can we recommend open-copyright forms to add to the current library? For example, I regularly use the C-SSRS (attached). Thank!
