Administer Forms During a Session

We're adding the ability to fill out any form on your client's behalf directly from their profile. For clinical assessments like the PHQ-9, reading questions aloud and recording answers yourself ensures accuracy, especially when clients might misinterpret what a question is asking.

Right now, forms can only be sent to clients to complete on their own. With this update, you'll be able to open any form in an "administer mode," record your client's responses as you talk through it together, and submit it to their chart. The completed form is saved the same way a client-submitted form would be, with a clear label showing it was submitted by the provider. If you're on a telehealth call, you can share the form on screen so your client can follow along.

This is especially useful for intake assessments, screening tools, and any situation where walking through the questions together gives you more reliable data to work with.

In‑session form administration is now live! 🎉

Providers can now fill out client forms during a live session (for example, while on a video call), instead of only having clients complete them in the Client Portal.

    How it works

    • From a Client Detail page, go to the Forms tab and click “Fill for Client” on any uncompleted sent form.

    • Or, in Practice Forms, open the form’s menu, click “Fill for Client”, select the client, and click Continue.

The form opens in a separate full‑screen tab optimized for in‑session use.

    Key details

    • Signature and payment/card forms still must be completed by clients.

    • No email notifications are triggered when a provider submits a form.

    • Provider‑submitted forms still appear securely in the Client Portal and are saved to the client’s chart with a clear “submitted by provider” label.
