Custom client-facing title for standardized assessments

Allow changing the title that the client sees for standardized assessments, even though the assessments themselves cannot be edited. Names like 'DSM-5 Level 1 Cross-Cutting Symptom Measure' or 'CORE-10 (Clinical Outcomes in Routine Evaluation)' can sound intimidating to a layperson and do not give them much usable information. Add a toggle switch option that presents the assessment to the client with a benign or generic title, such as Mental Health Assessment or something suitable to a non-professional.
