AI Private Note Generation

Add the ability to generate AI-assisted psychotherapy notes directly into the private notes section, or provide a "Save to Private Notes" button on generated summaries. This would let me:

• Leverage AI for drafting and clinical thinking

• Keep psychotherapy notes (reflections, direct quotes, mentalizing observations) completely separate from the medical record
