Align Headway note templates with stricter audit criteria

Revise Headway Templates

Update the Headway Intake Report and Progress Note templates to reflect Headway's new guidelines. Headway is conducting very strict audits with much more extensive criteria that must be included to pass, which the current Upheal templates do not include.
