ABA Template for Notes

Please make an ABA note template for BCBAs with more ABA friendly Subsections and information in the template. I even used the custom AI note and it still included therapist language and information in it that is not allowed in ABA notes.

📝 Control How People Are Named in Your Notes

We're giving you control over how people are referred to in your notes. Beyond first names and initials, you'll be able to use family roles like mother, father, or guardian, and set how you're referred to as the clinician. Right now the wording is fixed. If you write "clt" rather than "client," or "Coach" rather than "Therapist," or you think about a family by role, you're editing every note to match your own style. Setting it once means notes come out sounding like you wrote them. Settings stay separate for individual, couples, and family notes, so a change for family sessions doesn't rewrite how your individual notes read.

✂️ Split One Recording Into Separate Sessions

We're exploring the ability to split a single recording or session into separate sessions, each with its own transcript, note, and client. It happens more often than any of us would like: you forget to stop the recording between back-to-back clients, a couples session shifts into individual time with one partner, or a crisis walks in mid-hour and suddenly one recording holds two distinct clinical encounters. Today that means a messy transcript and a note that can't be filed cleanly. With splitting, you'd mark where one session ends and the next begins, assign each part to the right client, and get a proper note for each. The same foundation would cover the reverse problem too: generating two different notes from one session when the encounter genuinely covers two billable services or two family members' charts.

We're exploring two answers to the steady stream of modality requests: keep expanding the built-in list, and let you teach Upheal your specific approach. The built-in list keeps growing, and merging your requests here helps us see which modalities to prioritize next. But we also know the honest truth: the list of therapeutic approaches is effectively endless, and the way you practice AEDP or ISTDP or animal-assisted therapy is not quite the way anyone else does. So the more interesting direction is letting you describe your approach, your framework, your language, even the literature you work from, and having your notes and treatment plans reflect it. Comment with the modality you need and, just as useful, how you document it: what a good note in your approach looks like, what terms it uses, what it tracks across sessions. That's exactly the input that shapes how we build this.

📋 Your Notes Will Use the Diagnosis You Assigned

We're changing how diagnoses appear in your notes: when you've saved a diagnosis for a client, your documentation will use it, instead of inferring a diagnosis from that session's conversation alone. You told us why this matters, clearly and repeatedly. Many diagnoses rest on history that doesn't come up in every session: a past manic episode, when a trauma occurred, a pattern you established months ago. A note that reasons only from today's transcript can't know that, and you shouldn't have to correct your own documentation to reflect a diagnosis you already made. Your clinical judgment leads; the note follows it. If you haven't saved a diagnosis yet, nothing changes: Upheal will still suggest clinically relevant possibilities based on the session, and we're keeping that as a way to surface diagnoses worth considering, never as a replacement for your call.

📝 Treatment Plan Renewal Reminders

We're adding reminders for treatment plans that are due for renewal. Set how often plans should be reviewed for your practice, whether that's every 90 days or whatever your payer requires, and Upheal will flag the clients whose plans are coming due instead of you tracking dates in a spreadsheet. When a plan needs updating, you won't start from a blank page. Combined with treatment plan versioning, the update builds on the existing plan and everything documented since, so staying compliant becomes a review instead of a chore.

Quicker way to document progress notes

It would be great if we had a “form-based/checkbox” type of structure for documentation. A good example of this would be Headway’s template. I just feel it’s more efficient.

Automatic deletion of notes

As a new therapist coming in, I am concerned that all of my past notes are not being imported into Upheal. Having seen clients for 5 years, I’m only seeing notes from 1.5 years ago being uploaded, and not all of them. I ask the AI, and it tells me that maybe I have a setting for auto deletion. First, there are problems with auto deletion of notes, and definitely a problem if I am not aware of it in the first place, especially during the exporting period. If I am subpoeaned and I was assuming that the notes were there without having checked for them, then I would be in big trouble. The AI tells me that I should keep the notes for my own record. That is problematic as well because lawyers can argue that the clinical record of progress notes being in the same area and format are indistinguishable from process notes and therefore accessible during Discovery. On top of that, now I am realizing that I have to check every client record to make sure that the clinical record is complete and that is over 80 hours of work! Maybe even more! I argue that this is very problematic, and is enough to make me rethink using Upheal.

PDF Note Size max 50 MB

Is there any way to reduce the size of the PDF downloads for notes? I tried downloading a client summary into a PDF to share with a client I have at a different practice (SimplePractice Platform) and couldn’t upload the PDF due to size not being below 50 MB. Thanks!

🤖 Ask the AI Assistant About Your Imported Records

We're connecting the AI Assistant to the PDF files you upload from your previous EHR, so the years of client history you bring into Upheal stop being static documents and become something you can actually use. Once uploaded, the assistant can read those PDFs, reason across them, and answer questions about what's in them. Ask "what's the history of suicidal ideation with this client?" and the assistant pulls from the prior notes, intake assessments, and uploaded documents you brought over from SimplePractice or TherapyNotes. Ask it to summarize a client you haven't seen in six months before their next appointment, surface treatment history before writing a referral letter, or check whether a topic has come up before. The assistant works with the full picture of the client's care, including the parts that happened before they joined your Upheal practice. This pairs with self-service bulk upload, so bringing years of client history into Upheal stops being a manual back-and-forth with our team and becomes something you do in one sitting. The day you migrate is the day your imported records start working for you.

We're adding the ability to generate clinical documents directly from your session history and client summary. Starting with discharge summaries and clinical letters, Upheal will draft these documents using the data it already has, including session themes, treatment progress, and diagnosis, so you review and finalize rather than writing from scratch. Discharge summaries will pull from the full arc of treatment to produce a comprehensive overview. Clinical letters for disability documentation, court requests, or referrals will draw on relevant session details and your client summary to give you a solid first draft. You'll review, edit, and approve every document before it goes anywhere, the same workflow you're already used to with session notes.

Instead of creating workarounds with individual or couples sessions, you'll be able to set up a group, add members, and let Upheal capture the session with the full context of who's in the room. Group notes will capture overall themes and dynamics while noting individual client contributions, so each participant's chart gets the documentation it needs. Whether you're running a skills group, a process group, or a psychoeducation series, the notes reflect what actually happened rather than forcing group work into an individual session format.

We're adding treatment plan versioning so you can update a plan without losing the previous one. When it's time for a new version, your existing goals and objectives carry forward, and the prior version stays accessible in the client's chart. Each plan includes proper start and end dates alongside the clinician signature, giving you the documentation you need for compliance reviews and audits. When you create the next version, AI can suggest updated objectives and interventions based on what's happened in sessions since the last plan, and you stay in control of what carries over and what gets revised. For group practices, every provider follows the same versioning structure, making it easier to maintain consistent records across your caseload. Update what's changed, archive what's done, and keep a clean history of your client's treatment over time, without rebuilding from scratch every six months.
