Client Address Populating "First Name" Field

I’m not sure if this is happening to anyone else, but when my clients fill out the intake forms (Notice of Privacy Practices, Consent for Treatment), and enter their address, their first names are being overwritten by the addresses.

Is there a way to get the address to populate the correct field? I can do it manually, but it would be nice if I didn’t have to.
