Client Address Populating "First Name" Field

Feature Request

I’m not sure if this is happening to anyone else, but when my clients fill out the intake forms (Notice of Privacy Practices, Consent for Treatment), and enter their address, their first names are being overwritten by the addresses.

Is there a way to get the address to populate the correct field? I can do it manually, but it would be nice if I didn’t have to.

    Hi, I’m still having this issue. A client just completed the Telemental Health Consent form, and his address over-wrote his name.

    This looks like a bug rather than expected behavior, address data shouldn’t ever overwrite the first-name field. We’re flagging this to engineering to reproduce. Could you confirm which browser or device you’re on, and whether it happens every time or intermittently?
