Diagnoses in the client details...

Whenever I try to prompt the smart sections in templates or the upheal assistance to utilize the documented diagnoses located under client details it will not do it. It will come up with diagnoses or even differential diagnoses located only in the progress notes (which are there because it will not only reference the listed ones) and I have to keep correcting it. this gets frustrating because I try to prompt the template to do a justification for the diagnoses but it will come up with different ones.
