Emergency contact name and number field on client address page

Add an emergency contact name and number field to the client's main address and phone number page. The customer works with elderly clients and needs easy access to an emergency number when a client cannot be reached for a session. A form can hold that information, but in a crisis digging through forms is not efficient, nor is keeping a tablet with that info nearby when everything else needed is online.
