Form Feature

Hey folks :) It would be so nice to have an optional field in the form builder (the check marks or radio dial), when checked - to be able to have a typing field to explain more when a field is checked (I hope this makes sense). SimplePractice, as an example, when you have a check-box setup - you can also have a fillable line if you check that option.
