Hi Suzanne. We hear this request often. We have a team of clinicians, myself being one of them and we have also consulted with behavioral health coding consultants and brought this question to them. What we have come to know from auditing feedback and through this process is that auditors are typically not solely looking at one note for a justification of 90837, they are looking at the entirety of the chart, diagnoses, presenting symptoms and goal progress etc. We built our Golden Thread style of documentation to address the auditor concern for longer sessions as everything should support the narrative of complex cases.
A workaround to try would be to experiment with a smart section, you could try adding this section and placing it in your template. Reach out to us if you would like some support here.
