Multiple state licenses per provider with per-document selection

Allow adding and managing multiple professional licenses under a single provider profile for therapists independently licensed in multiple states. Providers could enter all their licenses, including the state, license type, license number, and expiration date. When signing a progress note, psychosocial assessment, treatment plan, or other clinical document, a dropdown menu should let the provider select the appropriate state license for that patient. The selected license should automatically appear alongside the provider's name, credentials, electronic signature, and signing date on the finalized document. It would also be helpful to assign a default license based on the patient's state, while allowing the provider to change it when necessary. For clinicians practicing across multiple states, this would eliminate the need to manually enter licensing information on every note, reduce documentation errors, and help meet applicable state licensing and payer documentation requirements. This is an important feature for independently licensed, multistate telehealth providers and would make Upheal a much more practical primary EHR for practices like the provider's. This was very helpful when the provider used Therapy Notes.
