Option to skip notes step in SimplePractice import

Improvement of Import from SimplePractice

When importing from SimplePractice, provide an option to skip the third step of bringing over notes and just bring over client names, addresses, etc. When a therapist moves from working at one practice to another, they can frequently bring their clients, but the files need to stay with the other practice. There should be an option to skip the last step of uploading the .zip file and just bring over the demographic information. Currently a blank .zip file has to be created in order to get the import to move on.
