Per-Client Custom Rate / Sliding Scale Field

There's no system-level way to set an individual client's session rate different from the standard fee schedule, which makes documented sliding scale arrangements harder to track and audit accurately. Suggested fix: add a per-client rate override field tied to the client's billing profile, so time-of-service charges automatically reflect the agreed sliding scale rate instead of relying on manual adjustment at each session.
