Per-service default to turn AI notes off for any service

Allow a clinician to set AI notes off by default for any service in service settings, not only for a hard-coded free consultation. That setting should be a choice for each service a clinician defines, so a consultation, intake, or any other non-clinical or first-contact service can be excluded by default, while clinical sessions where consent is on file remain on.
