Structured safety plan clients can access in portal

Provide a structured safety plan (Stanley-Brown or similar) that is built together with the client in session, saved to the chart with past versions kept, and visible in the client's portal or app. A safety plan only helps if the client can reach it in a hard moment. This supports client safety and gives clinicians clear documentation for risk management, which is especially important for teens.
