Telehealth Sign On Verification

We need to have a way that client's enter their identity and confirm where they are located.  The reason being I'm seeing audits of payors that are now requiring.

I do this for my notes:

Session was conducted via synchronous audio-video using the HIPAA-compliant Upheal telehealth platform. The clinician conducted the session from her private professional office located at 1885 The Alameda, Suite 100-F, San Jose, California 95126, and was physically located in California throughout the encounter.

Written informed consent for telehealth services is on file, and the client provided verbal consent to proceed with today’s session. Based on ongoing clinical assessment, the psychotherapy service remained medically necessary, and delivery via telehealth remained clinically appropriate.

Appropriate privacy, confidentiality, and security safeguards were maintained throughout the encounter. No significant technical, safety, privacy, or confidentiality concerns interfered with the provision of services. Emergency and telehealth procedures were reviewed as clinically indicated.

Client Identity and Current Telehealth Location

The client's full name and exact current physical location were verbally confirmed during the session. The client experienced significant and persistent technical difficulties that disrupted the session. The client's computer crashed at the beginning of the appointment, and he was subsequently locked out of his email. The client then attempted to join via his cell phone, which resulted in audio and video connectivity problems, including being unable to hear the therapist for a period. The client eventually was able to connect via his cell phone with audio and video. The therapist noted that using a phone could lead to bandwidth issues and suggested turning off the video if distortion occurred.

But this IS NOT sufficient though I would argue it is comprehensive.

They want a transcript of when and where the provider logged in and a transcript of when and where the client logged in from.
