Upload treatment plans as PDF or text
Allow uploading an existing treatment plan as a PDF or text file instead of requiring plans to be created directly in upheal. The way upheal's treatment plan section is set up is very tedious, and it currently requires inputting treatment plans in a very specific way.
Standalone case conceptualization separate from treatment plan
Provide the ability to create a case conceptualization outside of the full treatment plan formulation that can pull from all sessions and take into account additionally added information: a box where information can be typed and all or any notable sessions selected to also summarize. DETAIL: the case formulation should be generatable on its own, outside of the treatment plan, and updated on an as-needed basis with the ability to overwrite or keep prior formulations. It should pull from the full archive of session notes and/or transcripts or typed/dictated information and allow adding additional information. This would allow a case conceptualization to be completed for clients that have a history of sessions prior to utilization of upheal, and be a way for clinicians to utilize upheal as part of the personal process, rather than only the formal documentation. This could also be a beneficial tab for the clinician and assistant to: a) collaboratively identify additional areas warranting assessment, for instance b) provide recommended modalities or interventions beyond formal documented treatment planning (more session-to-session or just overall long-term strategizing before doing the official collaborative treatment plan), or c) support the drafting of case presentation and clinical questions so that the clinician can quickly have a summary to present for case consults or supervision. The assistant may also be able to identify things going well and areas of improvement for the clinician, in which case this tab could be beneficial for this. It should be a "private/personal/process notes" thing, rather than a part of the formal record, which some EHRs have done. Likely usage: most during initial sessions when assessment is the primary focus alongside rapport building, updated leading up to any formal collaborative treatment planning appointment that requires the official treatment plan document, and to prepare for case consults/supervision as needed — possibly usage 3x in the first month with subsequent 1x usage every 3-6 months for treatment planning and intermittent usage for case consultation/supervision. Overall, the core need is to update this without updating the txplan, with the additional functionality described above as a bonus.
Update existing treatment plan without deleting it
Allow updating an existing treatment plan instead of having to delete the current plan to generate a new one. Use the existing plan and add additional sessions or modify the existing sessions rather than deleting the entire plan for a new one.
Electronic signing and locking for chart notes
Add an option to electronically sign and lock chart notes (non-session notes). Unlike session progress notes, chart notes don't appear to have an option to be electronically signed and locked, which is different from most EHR platforms. Chart notes are used for important clinical documentation outside of sessions, including case closures, outreach attempts, and continuity-of-care documentation, and leaving them unsigned and editable is a concern, particularly for case closures, since records should be properly finalized and meet clinical, legal, and audit requirements if ever reviewed. Upheal's suggested workaround was to "download the note as a PDF and save it in your records", but the point of using Upheal as the EHR is for it to save notes and records. Requested as an optional feature for chart notes to ensure notes are in their completed form and secure.
Report of unsigned notes
Reports of unsigned notes Provide a report that can be run listing unsigned notes.
Dictation not working when dictating a note
The dictation feature does not work when trying to dictate a note directly. It has been tried across multiple days and devices with the same result.
Free-text notes on appointments for no-shows and sick clients
Allow typing a short free-text note on an appointment, for example to mark a no-show or a client who was sick and did not come. Currently there is no place to just type in notes.
Read notes out loud
Allow the note to be read out loud. The chrome extension to read it isn't good at finding the correct part of the website to read.
Default note template per service type and session type
Set Default Note Template by specific service types and the session type. For Example: Intake & Individual vs Intake & Couples then Therapy Session & Individual vs Therapy Session & Couples Allow setting a default note template based on the specific service type and the session type, so different combinations can have different defaults. For example: Intake & Individual vs Intake & Couples, then Therapy Session & Individual vs Therapy Session & Couples.
ABA Template for Notes
Please make an ABA note template for BCBAs with more ABA friendly Subsections and information in the template. I even used the custom AI note and it still included therapist language and information in it that is not allowed in ABA notes.
📝 Control How People Are Named in Your Notes
We're giving you control over how people are referred to in your notes. Beyond first names and initials, you'll be able to use family roles like mother, father, or guardian, and set how you're referred to as the clinician. Right now the wording is fixed. If you write "clt" rather than "client," or "Coach" rather than "Therapist," or you think about a family by role, you're editing every note to match your own style. Setting it once means notes come out sounding like you wrote them. Settings stay separate for individual, couples, and family notes, so a change for family sessions doesn't rewrite how your individual notes read.
Supervision Note Template
It would be great to have a note template specific to doing therapeutic supervision of associate therapists!
✂️ Split One Recording Into Separate Sessions
We're exploring the ability to split a single recording or session into separate sessions, each with its own transcript, note, and client. It happens more often than any of us would like: you forget to stop the recording between back-to-back clients, a couples session shifts into individual time with one partner, or a crisis walks in mid-hour and suddenly one recording holds two distinct clinical encounters. Today that means a messy transcript and a note that can't be filed cleanly. With splitting, you'd mark where one session ends and the next begins, assign each part to the right client, and get a proper note for each. The same foundation would cover the reverse problem too: generating two different notes from one session when the encounter genuinely covers two billable services or two family members' charts.
🧠 More Modalities, Including Your Own
We're exploring two answers to the steady stream of modality requests: keep expanding the built-in list, and let you teach Upheal your specific approach. The built-in list keeps growing, and merging your requests here helps us see which modalities to prioritize next. But we also know the honest truth: the list of therapeutic approaches is effectively endless, and the way you practice AEDP or ISTDP or animal-assisted therapy is not quite the way anyone else does. So the more interesting direction is letting you describe your approach, your framework, your language, even the literature you work from, and having your notes and treatment plans reflect it. Comment with the modality you need and, just as useful, how you document it: what a good note in your approach looks like, what terms it uses, what it tracks across sessions. That's exactly the input that shapes how we build this.
📋 Your Notes Will Use the Diagnosis You Assigned
We're changing how diagnoses appear in your notes: when you've saved a diagnosis for a client, your documentation will use it, instead of inferring a diagnosis from that session's conversation alone. You told us why this matters, clearly and repeatedly. Many diagnoses rest on history that doesn't come up in every session: a past manic episode, when a trauma occurred, a pattern you established months ago. A note that reasons only from today's transcript can't know that, and you shouldn't have to correct your own documentation to reflect a diagnosis you already made. Your clinical judgment leads; the note follows it. If you haven't saved a diagnosis yet, nothing changes: Upheal will still suggest clinically relevant possibilities based on the session, and we're keeping that as a way to surface diagnoses worth considering, never as a replacement for your call.
📝 Treatment Plan Renewal Reminders
We're adding reminders for treatment plans that are due for renewal. Set how often plans should be reviewed for your practice, whether that's every 90 days or whatever your payer requires, and Upheal will flag the clients whose plans are coming due instead of you tracking dates in a spreadsheet. When a plan needs updating, you won't start from a blank page. Combined with treatment plan versioning, the update builds on the existing plan and everything documented since, so staying compliant becomes a review instead of a chore.
Quicker way to document progress notes
It would be great if we had a “form-based/checkbox” type of structure for documentation. A good example of this would be Headway’s template. I just feel it’s more efficient.
