We're changing how diagnoses appear in your notes: when you've saved a diagnosis for a client, your documentation will use it, instead of inferring a diagnosis from that session's conversation alone.
You told us why this matters, clearly and repeatedly. Many diagnoses rest on history that doesn't come up in every session: a past manic episode, when a trauma occurred, a pattern you established months ago. A note that reasons only from today's transcript can't know that, and you shouldn't have to correct your own documentation to reflect a diagnosis you already made. Your clinical judgment leads; the note follows it.
If you haven't saved a diagnosis yet, nothing changes: Upheal will still suggest clinically relevant possibilities based on the session, and we're keeping that as a way to surface diagnoses worth considering, never as a replacement for your call.
